Escalating violence in El-Fasher worsens Sudan’s humanitarian crisis and fuels disease outbreaks

The number of displaced people in Tawila, a town located about 60 kilometers from El-Fasher, has tripled this year as intense fighting continues to engulf North Darfur. According to Ugochi Daniels, Deputy Director General for Operations at the International Organization for Migration (IOM), the violence has also caused El-Fasher’s population to shrink by 62% in 2025 alone.

Daniels warned that the growing number of displaced persons poses serious humanitarian challenges, adding that the situation is now part of what many aid workers have described as the world’s largest displacement crisis.

Escalation of the Conflict

The sharp increase in displacement is linked to intensified fighting between the Sudanese army and the Rapid Support Forces (RSF), both of which are battling for control of El-Fasher — the military’s last major stronghold in the Darfur region.

The conflict between the RSF and the army began in 2023 after tensions erupted between the two former allies, who were initially tasked with overseeing a democratic transition following the 2019 uprising.

Humanitarian Toll and Food Insecurity

The ongoing war has claimed at least 40,000 lives, according to the World Health Organization (WHO), and displaced around 12 million people. More than 24 million Sudanese are facing acute food insecurity, according to the World Food Programme (WFP).

“North Darfur remains the epicenter of the crisis here in Sudan, and we need peace to be able to access the most difficult places where the needs are the greatest,” Daniels said.

Daniels noted that providing essential services for the rapidly growing displaced population has become increasingly difficult as health care and infrastructure continue to collapse.

The mass displacement has severely restricted access to clean water, proper nutrition, and medical services, worsening the humanitarian crisis in the region.

Disease Outbreaks Deepen the Crisis

Haitham Ibrahim, Sudan’s Health Minister, said that while health services have been partially restored in 60% of hospitals in army-controlled areas, diseases such as cholera, malaria, and dengue fever remain widespread.

Speaking at the World Health Organization’s Regional Committee meeting in Cairo, Ibrahim said dengue has spread to at least 14 provinces. WHO reported that since July 2024, Sudan has recorded 113,629 cholera cases and 3,029 deaths.

Aid Delivery Hampered by Insecurity

The minister also revealed that medical aid reaching communities remains far below demand due to widespread looting and attacks on aid convoys. This insecurity has made it difficult for humanitarian agencies to operate effectively in the war-torn areas.
